We understand that you may have many questions when choosing a doctor, and we want to make the process as easy as we can for you. In order to assist you, we have dedicated this web page to provide you with the information that you are looking for.

How to make an appointment

To make an appointment, please call (318) 226-9441 for the 2121 Line Avenue office or (318) 798-0759 for the 8575 Fern Avenue location. Our physicians make every effort to see you at your appointment time. Most days, we are able to offer same day appointments. Emergencies and unexpected events may occasionally prevent the physician from maintaining his appointment schedule. Rarely, if called away for an emergency or delayed from surgery, your appointment may need to be rescheduled. We will do everything we can to notify you as soon as possible if this should occur.

If you need to be seen urgently, please contact our office and one of our physicians will usually be able to see you the same day. Walk-ins are accepted, but please understand that your wait time may be longer as others have scheduled appointments.

Appointment times

Monday through Friday, 8:30 a.m. to 4:15 p.m. The offices are closed on Saturday and Sunday. To reduce your wait for your appointment with us, please print out and complete the following forms and bring them to the office with you.

Upon your arrival, we will ask to copy your insurance card and the driver’s license of the patient or guarantor. All co-pays and deductible’s will be collected at the time of service.

Emergencies

If you have an emergency, please call 9-1-1 or go to the emergency room of your choice. Our normal office hours are 8:30 a.m. to 5:00 p.m., Monday through Friday.

If you have a non-life threatening emergency during this time, please call our office at (318) 226-9441 and we will attempt to advise you as to the best location to see you as quickly as possible.

If you need a prescription refill or routine office question, please call during office hours so we may review your chart and best treat you.

If you have recently had surgery and are having a problem, please call our answering service at (318) 226-9441 to speak to a physician.

Insured Patients

You are financially responsible for the services provided to you. As a courtesy to you, we will file a claim on your behalf with your insurance company. If you fail to provide insurance information at the time of service, you will be considered an uninsured patient for that visit. Insured patients are responsible for paying applicable co-payments, co-insurance and deductible amounts at the time of service.

Worker’s Compensation

The worker’s compensation carrier must verify worker’s compensation visits. We will need a letter from the carrier stating that this is a worker’s compensation injury and where medical claims need to be submitted.

Financial Counseling

Financial counseling is available for patients who request to speak with a billing clerk. Our billing clerk will help patients establish reasonable payment plans and can give information regarding the cost of services provided by our physicians.

Our practice offers several payment options; cash, check, money order, Visa, MasterCard and Discover. We also participate in the CareCredit Network. We do not want to cause any financial hardship or embarrassment to any patient. Let us assist you if you have any question or problems with your account.

All charges are due and payable within 30 days from the date of service. If your payment is late, or if you have not previously made financial arrangements, we will mail a statement indicating that payment is due. Patients unwilling to comply with our financial policies will limit credit for future services. Any outstanding patient balances should be paid prior to any follow-up visits.

Our practice firmly believes that a good physician/patient relationship is based on understanding and open communication. Our staff has been instructed to make every effort available to you to clarify any misunderstanding you have concerning your balance. We hope to avoid any disagreement over payment for professional services. If you have any questions concerning our policies or need assistance, please contact us immediately.

Confidentiality and Medical Records

Your medical records contain information which is confidential and will not be released without written authorization by you or your guardian, unless subpoenaed by the court system. Request for copies must be submitted in writing and contain the full name and address of the party to receive the records, as well as your full name, address, social security number and date of birth.

Prescriptions

Request for prescription refills should be made during normal office hours (8:30 a.m. through 5:00 p.m., Monday through Friday). Please note your supply of medications preceding holidays, weekends, or after hours, as refills are difficult or impossible during these times because the emergency on-call physician does not have access to your records.
